Depression overcome!

Now for the conventional stuff!

       Depression

It is portrayed as an overwhelming feeling of sadness with an inability to cope. It affects every part of your life as you struggle to get through each day. To add to the problem there is also the unspoken stigma of admitting you are depressed.

What are the symptoms?    

Mood swings, sleep disturbances, Dullness of thought/speech/movement, Anxiety, Tearfulness, Short temper, Lethargy, Exhaustion, Loss of joy, Feeling of emptiness, Lack of concentration, Difficulty making decisions, Forgetfulness, Negative thoughts about the future, Guilt, Loss of identity, Self blame, Low self esteem, Hopelessness, Despair, Sense of failure, Agonising loneliness, Preoccupation with illness, Loss of appetite,Lack of libido, Feeling useless/inadequate/bad, Self hatred, constant questioning of own thoughts/actions, Constant need for reassurance, Feeling vulnerable, Oversensitive to criticism, Suicidal thoughts, Physical aches and pains

Depression normally follows a pattern which goes from feeling blue to the clinical symptoms listed above. It would be unusual to suffer from every symptom. People with severe depression may experience suicidal feelings, stop eating or drinking and suffer from delusions or hallucinations.

Who suffers from Depression?                                                                                                        

Between 7 and 12 % of men suffer from diagnosable depression. 20- 25% of women. (Post natal depression in women makes this a higher number). Costs in the UK alone stand at about £8bn a year in medication, benefits and lost working days.  

Different types of depression

There are different types of depression, including clinically diagnosed depression, manic depression and post-natal depression.

Manic depression is marked by extreme mood swings, between highs when a person experiences excessive energy and optimism and lows when they may feel total despair and lack of energy. It is often treated with lithium or, in extreme cases, electro-convulsive therapy.

Post-natal depression can occur from about two weeks after the birth of a child to two years after and differs from the mood swings suffered by many in the first few days after the child is born.

Other forms of depression include SAD (Seasonal Affective Disorder) which is thought to be associated with the approach of winter and lack of sunlight.

What causes Depression?

It is most commonly thought that it is a combination of the following:

Major depressive illness is linked to some form of chemical imbalance in the brain caused by a neuro-chemical or hormonal imbalance.

Certain styles of thinking. It is also thought that people with low self-esteem, a pessimistic outlook on life and difficulty coping with stress are more prone to depression.

A result of unfortunate experiences in life such as bereavement, chronic illness, relationship problems and financial difficulties.

Often runs in families, suggesting a genetic component, but it may be triggered by stressful events.

What is the available treatment offered by modern medicine?

Doctors and psychiatrists have been treating cases of depression with psychotherapy, drugs and in extreme cases hospitalisation. Besides psychotherapy and a course of anti-psychotic drugs, people admitted to hospital may be offered or forced to undergo electro-convulsive therapy. This involves applying electric currents to the brain. Some psychiatrists are against its use, particularly without the patient's consent, but others believe it can be effective in dealing with life-threatening depression.

As with all drugs some people react badly to antidepressants, whilst side effects can seem quite mild in others. Because no one antidepressant has been proven to be any more effective than any other, the choice of which drug to prescribe often rests on their different side effects! The popularity of SSRIs (selective serotonin reuptake inhibitors) is in part due to their apparent "safety" over more toxic drugs when used improperly. Some of the drugs are extremely toxic in overdose. In addition to other dangers, there is also an established direct link between suicide and violent behaviour and the use of SSRIs.

The reason there are so many side effects with antidepressants is due to the lack of understanding about how antidepressants, and depression, affect the brain.  This can vary from patient to patient. Drug companies themselves admit that they don't fully understand how the drugs work!

Side effects from the different antidepressants may include:

     Dry mouth, Urinary retention, Blurred vision, Constipation, Sedation (can interfere with driving or operating machinery), Sleep disruption, Weight gain, Headache, Nausea, Gastrointestinal disturbance/diarrhoea, Abdominal pain, Inability to achieve an erection, Inability to achieve an orgasm (men and women), Loss of libido, Agitation, Anxiety.
